Please take a moment to follow the link below and complete the Hand In Hand Spring 2010 Evaluation sheet.  We would like to know what you thought of the program, and would like to know of any concerns within the program or things you thought went well with the program.

The evaluation is in PDF format, so you can either print it out and Fax it to the office at 630-377-2022, or you can print it out and either mail the evaluation to our office or drop it off at our office!  Whichever is more convenient for you!

Once again, we appreciate you going through our Hand In Hand program, and we genuinely hope it got your child interested in the game of soccer!
